If you are new to Sofunk® Dance Fitness, please complete and submit the PAR-Q (Physical Activity Readiness Questionnaire), before booking any classes. You are only required to complete this form once, however you must notify Sharon Brown Fitness of any changes to your health and medical circumstances.
